Thanks for visiting our online booking page! Please note: At this time, we are only allowing online booking for self-pay chiropractic visits. Self-pay does not include using health insurance and only applies to those using “date of service discounts”, ChiroHealthUSA memberships, and prepaid plans. It does NOT include those who use their health insurance, even if you pay a copay or deductible amount toward your insurance on the same day as your visit! Do NOT book an online appointment if you want to use your health insurance! At this time, you must call us for health insurance visit scheduling so we can be certain all parameters for care plans, paperwork, etc. are met.

Instructions for chiropractic self-pay visits: If it has been more than one year since your last visit here, you MUST call our office to schedule! If it has been less that one year, you can click the ‘Chiropractic Adjustment’ blue box under the ‘Book by Service’ heading to see available appointments for both doctors. If you would like to see appointments available for just one doctor, click on their photo first.

You can now also schedule a Sports Physical with either doctor, and a DOT Physical with Dr. Sam.

Please call if you are unable to find an appointment time that works for you. We may be able to find something that will work for you that doesn’t show as an option in online booking.
